Sample Quality Assurance Statement of Work and Reporting Templates
In accordance with statewide policy, direct quality assurance oversight is provided primarily through the use of third party independent quality assurance contractors.
The sample quality assurance statement of work appendices, and reporting templates attached are collaborative work products developed in partnership with multiple state agencies. The documents were originally developed to ensure that consistent quality management and risk assessment processes and standards are applied to the oversight of each major IT project.
Special thanks goes to agency staff working for the Oregon Department of Human Services (Office of Information Systems - Strategy Systems Initiatives), the Oregon Department of Transportation - (Information Systems - Transportation Application Development) and assigned counsel from the Oregon Department of Justice (Business Transaction Unit).

Statewide IT Quality Assurance (QA) Price and Services Agreements (PSA)
The Oregon State Procurement Office has entered into Statewide Price Agreements with eleven (11) contractors under which State and local government organizations ("Authorized Purchasers") can execute Work Order Contracts ("WOC") to acquire diverse Information Technology (IT) Quality Assurance (QA) and related professional services for review oversight and risk management ("QA Services"). Authorized Purchasers may select a particular QA Services Contractor for a single IT project, phased IT project or portfolio of IT projects either through an intelligent rotation system ("IRSYS") process for State Agency Major IT Projects or a "best value" competitive ("BVC") process within the pool of Price and Services Agreement ("PSA") holders for all other engagements.
The State Procurement Office and EISPD conducted an informational forum concerning the purpose and use of these new agreements on May 15, 2009. The 11 Contractors introduced themselves at the forum through company representatives and the submission of slides providing information about their firms, including what they do in the IT QA services realm and their experience in providing these services. The attached PowerPoint document provides a detailed overview of the agreements and reflects the information shared at the forum. The State Procurement Office and EISPD intend to periodically refresh the overview presentation.
