Accident Report Packet for State Owned Vehicles
 
What´s in a packet?
 
Each state vehicle should have an "Accident Report Packet" in the glove box. If the state vehicle is damaged in a crash, read the instructions on the packet jacket.  Inside you will find:
  • Driver Information Sheet (doc) - “Blue 8.5x11”
    • At the scene, use the blue sheet to collect information about the state driver and the other driver.
    • The state driver should keep this form for use later to fill out the DMV form.
  • Witness card (doc) – Golden 3.5x 6 postcard
    • At the scene, record names and addresses of any witnesses to the crash.
  • Information you must provide the other driver as required by state law.
Need a new packet?
 
If you have a permanently assigned vehicle from DAS-Motor Pool:
  • Check to see if the Accident Report Packet is in the glove box. Make sure it has all the forms.
  • Request replacements from DAS-Motor Pool. Call them at (503) 378-4377 or (800) 378-0077.
If you drive a state vehicle not owned by DAS, ask your Fleet Manager or Risk Coordinator for an Accident Report Packet.
