Multicultural Health Mini-Grant Program

 

The Office of Multicultural Health and Services offers mini-grants. Organizations that provide outreach, health education, and health promotion to under-served, ethnic and racial communities of color are welcome to apply.

 

Grants will be awarded up to $5,000.00 per recipient. Funds will be allocated on a rolling basis, and applications will be reviewed as they are received.

 

What is the deadline?

We are encouraging applications to be submitted as soon as possible.  Applications will be accepted through July 30, 2009.

 

Applications Should Include:

1. Completed application form

2. Proposed project & budget.

3. Copy of signed W-9 form

Application Time Line:

June 8, 2009                              Funding Announcement, grant

                                                applications accepted

June 16, through July 30, 2009    Application due date

June 19, 2009                            Begin notification of grantees

June 25, 2009                            Begin disbursement of grant funds

 

How will applications be evaluated?

Applications must demonstrate how proposed projects will strengthen existing or develop new local health outreach projects, and/or demonstrate how they will improve health outcomes among Oregon's diverse, racial and ethnic populations.

 

Scoring: 

Applications will be scored and ranked by the OMHS administrator as follows:

 

    Relevance of proposed activity to OMHS Mission: 20%

    Demonstration of community engagement/partnerships: 20%

    Proposed Activity Description: 45%

    Project Methods/Evaluation Plan: 10%

    In-kind Contribution or Cash Match: 5%
