There have been some questions about what Oregon law requires on a custom feed label. We hope this quick summary will help. A custom feed is a formula sold to only one final consumer. Custom feeds are exempt from product registration. However, the feed mill producing custom-formula feeds must be licensed with the department each year. Labels for Custom Feeds must include the following information: - Custom Mix Feeds - | - Medicated - | - Non-Medicated - | Made for:_______________ Date sold:______________ The word "MEDICATED" Drug purpose statement Directions for use Any warning or caution statements Manufactured by: Name & address of manufacturer Quantity statement | Made for:______________ Date sold:______________ Manufactured by: Name & address of manufacturer Quantity statement |

(ORS 633.028 (5)) Batching records and formulas must be retained for two years after the feed is manufactured.
