There have been some questions about what Oregon law requires on a custom feed label. We hope this quick summary will help.
A custom feed is a formula sold to only one final consumer. Custom feeds are exempt from product registration. However, the feed mill producing custom-formula feeds must be licensed with the department each year.
Labels for Custom Feeds must include the following information:

The invoice must contain the following information and this information must also be sent with the delivery.
1. Name and address of the mixer
2. Name and address of the purchaser
3. Date of sale
4. If medicated: The amount of drug present and directions and warnings against misuse of the feed. (from the Food Additive Compendium.)
The mixer must keep a record of the above information on hand for at least two years. Mixer must provide batching records showing how much of each ingredient was used if requested by the customer. (ORS 633.028 (5)) Batching records and formulas must be retained for two years after the feed is manufactured.

In addition to the information above on the invoice,
Each bag or container must have a label (or tag) identifying the customer either by name or by order number.
Each sack of medicated feed must also show
1. the purpose of the drug ingredient,
2. the name of each active drug,
3. the amount of each drug used, and
4. any special feeding directions, withdrawal, or caution statements shall also be on the label.
